We are committed to maintaining a safe, supportive, and positive environment for all Sigma Alpha Iota members. This incident report form allows for the confidential reporting of any incidents, to allow for prompt and appropriate follow-up by fraternity leadership. The first step of reporting should be done by contacting local chapter leadership, collegiate chapter advisors, and/or the Province Officer.
What Qualifies as an Incident?
Incidents may include, but are not limited to, conduct violations, hazing, behavioral concerns, discrimination, harassment, or any activity that compromises the values and policies of Sigma Alpha Iota International Music Fraternity.
Hazing is defined as any action or situation that recklessly or intentionally endangers mental or physical health or any action taken or situation created which produces mental or physical discomfort, embarrassment, harassment or ridicule.
Confidentiality and Reporting Process
We prioritize the confidentiality and integrity of all submitted reports. Once a report is filed, it is reviewed by authorized personnel within the Fraternity. Further action is taken based on the nature and severity of the incident, with regular follow-up communication to the reporting party (if contact information is provided). If someone's immediate safety is at risk, please contact your local law enforcement before filing out this form.
